Liability Waiver & Assumption of Risk

I, the undersigned parent or legal guardian, understand that participation in Summer Safari Camp activities involves inherent risks, including but not limited to boating hazards, slips and falls, water-related activities, sun exposure, insect bites, contact with marine wildlife, changing weather conditions, and other risks associated with outdoor coastal recreation.

I voluntarily allow my child to participate with full knowledge of these risks and assume all risks, known and unknown, including those arising from the ordinary negligence of Mason Southerly, LLC, DBA Yellow Boat Tours, its owners, employees, contractors, and representatives.

In consideration of my child being allowed to participate, I release and hold harmless Mason Southerly, LLC, DBA Yellow Boat Tours and its representatives from any claims, liabilities, damages, injuries, or expenses arising from participation in camp activities.

I certify that I am the parent or legal guardian of the child listed below and have authority to sign this agreement.

Emergency Medical Authorization

By signing below I am agreeing that in the event of illness or injury, I authorize Mason Southerly, LLC, DBA Yellow Boat Tours and its representatives to obtain emergency medical treatment for my child if I cannot be reached immediately. I also am agreeing that I understand I am responsible for any resulting medical expenses by signing below.
